The Effects of Cdx-2 Polymorphism in the Promoter Region of the Vitamin D Receptor Gene on Bone Mineral Density in Postmenopausal Korean Women

Abstract
Objectives: The intestine-specific transcription factor Cdx-2 played an important role in the transcription of the human vitamin D receptor(VDR) gene. The objective of this study was to investigate the relationship between the Cdx-2 polymorphism in the promoter region of the vitamin D receptor gene and the bone mineral density(BMD) in postmenopausal Korean women.

Methods: We enrolled 150 healthy postmenopausal women. The bone mineral density at the lumbar spine and proximal femur were measured by dual energy X-ray absorptiometry. The polymorphism was analyzed in the core sequence 5¡¯-ATAAAAACTTAT-3¡¯ in the Cdx-2 binding site in the VDR gene promoter(Cdx-2 polymorphism).

Results: In 150 postmenopausal women, 30 were genotype Cdx-A(adenine at -3731 nucleotides relative to the transcription start site of human VDR gene 5¡¯-ATAAAAACTTAT-3¡¯), 54 were genotype Cdx-G(guanine at -3731 nucleotides, 5¡¯-GTAAAAACTTAT-3¡¯), and 66 were genotype Cdx-AG(heterozygote). A significant intergroup difference in BMD was not found at the lumbar spine and proximal femur among the different genotypes(p=ns). There was no difference in distribution of the Cdx-2 genotype between the osteoporosis and control group(?2=1.76, P=0.415). Even in the individuals with a low calcium intake(daily calcium intake less than 400mg), there was no association of Cdx-2 genotype with lumbar spine BMD or proximal femur BMD.

Conclusion: These results showed that there was no significant effect of Cdx-2 polymorphism in the promoter region of the vitamin D receptor gene on BMD in postmenopausal Korean women.
